Acknowledge a Facility or Agency

Oklahoma is filled with caregivers of all ages. Some wear capes and others don’t. Nursing homes, Assisted Living, Hospice Houses, Home Care agencies, families, and friends all participate in the act of caregiving. There is no professional experience or special training that can prepare someone for the toll it can take to be a caregiver because it always ends in grief. At times caregivers can lose their sense of identity, and can be time-consuming, and energy-draining. No matter how much you love someone, it may lead to resentment, caregiver fatigue, and depression. It is not uncommon for caregivers to neglect their well-being when it means putting their loved ones first. OSHO wants to acknowledge the sacrifices that have been made on behalf of others. If you are, or know someone/agency, please use the form below to honor them. OSHO puts on a Rose Ceremony every year and we would like to use your acknowledgment to honor them.

This form takes approximately 3-7 minutes to complete.
